Indications/Uses
Short-term treatment of active duodenal ulcer. H. pylori eradication to reduce risk of duodenal ulcer recurrence. Maintenance of healed duodenal ulcers. Short-term treatment of active benign gastric ulcer. Healing & risk reduction of NSAID-associated gastric ulcer. GERD. Maintenance of healing of erosive esophagitis. Long-term treatment of pathological hypersecretory conditions including Zollinger-Ellison syndrome.
Dosage/Direction for Use
Adult Duodenal ulcer 15 mg once daily for 4 wk. Maintenance of healed duodenal ulcer: 15 mg once daily. H. pylori eradication to reduce risk of duodenal ulcer recurrence Triple-therapy: 30 mg bd + amoxicillin 1 g bd + clarithromycin 500 mg bd for 10-14 days. Dual-therapy: 30 mg tds + amoxicillin 1 g tds for 14 days. Benign gastric ulcer 30 mg once daily for up to 8 wk. NSAID-associated gastric ulcer Healing: 30 mg once daily for 8 wk. Risk reduction: 15 mg once daily for up to 12 wk. Symptomatic GERD 15 mg once daily for up to 8 wk. Erosive esophagitis 30 mg once daily for up to 8 wk. Maintenance of healing of erosive esophagitis 15 mg once daily. Pathological hypersecretory conditions including Zollinger-Ellison syndrome 60 mg once daily. Childn 12-17 yr Non-erosive GERD 15 mg once daily for up to 8 wk. Erosive esophagitis 30 mg once daily for up to 8 wk. Childn 1-11 yr, >30 kg Short-term treatment of symptomatic GERD & erosive esophagitis 30 mg once daily for up to 12 wk, ≤30 kg Short-term treatment of symptomatic GERD & erosive esophagitis 15 mg once daily for up to 12 wk.
Administration
Should be taken on an empty stomach: Place tab on the tongue to disintegrate & swallow w/ or w/o water. Alternatively, disperse tab in a small vol of water & administer via oral syringe/nasogastric tube. Flush syringe/nasogastric tube w/ water & administer.
Special Precautions
Gastric malignancy. Discontinue if acute interstitial nephritis develops. Clostridium difficile associated diarrhea (CDAD). Osteoporosis-related fractures of the hip, wrist or spine. Cutaneous lupus erythematosus (CLE) & SLE. Cyanocobalamin (vit B12) deficiency. Hypomagnesemia. Monitor Mg levels prior to initiation of treatment. Concomitant use w/ methotrexate. Severe hepatic impairment. Pregnancy & lactation. Neonate to <1 yr.
Adverse Reactions
Abdominal pain, constipation, diarrhea, nausea.
Drug Interactions
Reduced absorption of drugs where gastric pH is a determinant of bioavailability (eg, ampicillin esters, ketoconazole, atazanavir, nelfinavir, Fe salts, erlotinib, & mycophenolate mofetil). Increased absorption of digoxin. Increased INR & prothrombin time w/ warfarin. Increased whole blood levels of tacrolimus. Minor increase in clearance of theophylline. Elevated & prolonged serum levels of methotrexate. Serious adverse reactions w/ clarithromycin.
